• Describe the importance of DVT prophylaxis in the hospitalized adult patient to include appropriate risk assessment
• Review the new anti-thrombotic website and the protocols and guidelines including heparin, low molecular weight heparin, vitamin K and direct thrombin inhibitors
• Describe the appropriate transition and monitoring of a patient from heparin or a low molecular weight heparin to warfarin
• Explain the importance of standardized education and monitoring for patients on appropriate anti-thrombotic therapy
